Public Service Announcement: Complete the City Census As Soon As Possible

In accordance with Massachusetts General Law Chapter 51, Section 4, the 2017 Annual City Census was be mailed in January to every household in the City of Revere. Upon receipt of the form review it for accuracy and make changes as needed. Sign and date the form and return it in the envelope as soon as possible, even if no changes are needed.
Census forms will not be sent to the residents in nursing homes, or senior citizen housing. Instead the administrators of these facilities are required to provide us with the necessary information.
We will not be going door to door to obtain this information.
If you have not received your census form, call the Election Department at (781) 286-8200 and a replacement form will be sent to you.
Failure to respond to this mailing shall result in removal from the active voting list and may result in removal from the voter registration rolls.

If you return your form using the United States Postal Service you must place a stamp on the envelope.
Compliance with this State requirement provides proof of residence when filing for State or Local benefits.
ATENCIÓN RESIDENTES DE REVERE
Conforme el Capítulo 51, Sección 4 de la Ley General de Massachusetts, en enero se enviará por correo postal el Censo Anual 2017 de la Ciudad a todos los hogares en la Ciudad de Revere. Cuando reciba el formulario, léalo para corroborar su exactitud y realice los cambios que sean necesarios. Coloque su firma y la fecha en el formulario y devuélvalo en el sobre provisto dentro de los (10) días de haberlo recibido, incluso si no necesita cambiar nada.
Los formularios del censo no serán enviados a residentes en hogares de ancianos o viviendas para jubilados. Los administradores de estos establecimientos están obligados a proporcionarnos la información requerida.
No iremos puerta por puerta para obtener esta información.
Si para el 3 de febrero no ha recibido su Formulario del Censo, llame al Departamento Electoral al (781) 286-8200 y se le enviará un reemplazo para.
No responder a esta correspondencia, dará lugar a la remoción de su nombre de la lista de electores activos, con la posible eliminación posteriormente de su nombre del padrón electoral.

Si devuelve el formulario a través del Servicio Postal de los Estados Unidos deberá colocar una estampilla en el sobre.
